What causes misted (blown) double glazing?

Double-glazed windows are sealed units: two panes of glass with a gap in between. Over time the seal can fail, letting moist air in. That moisture condenses on the inside of the glass — the foggy, misty look you can't wipe off. It's a sign the sealed unit has gone, but the frame is usually perfectly fine.
